ABOUT THIS JOB:

The Department of Case Management at The ARM (American Regional Medical) Hospital of Charleston SC is seeking a full-time case management manager.

A case management manager position at The ARM Hospital of Charleston SC involves:

  • Serving as a liaison between patients with long-term health issues, relevant medical staff, and patient families
  • Coordinating patient long-term care, including drafting and monitoring treatment plans
  • Helping patients make informed decisions
  • Maintaining client records, analyzing results, and communicating patient progress to relevant, authorized parties

QUALIFICATIONS:

Required

  • BS in nursing or BSW in social work
  • Current relevant licensure by the state of South Carolina
  • Eligible for certification as case manager
  • At least one year of experience in nursing or social work
  • Ability to work independently with limited supervision
  • Ability to work as part of a team

Preferred

  • Masters degree in nursing or social work with a concentration in case management
  • Current certification as case manager
  • At least 3 years of successful experience in case management

The successful candidate for this position will be subject to a pre-employment background check.
